1. PLAYER REGISTRATION FORM
Basic player information, parent/guardian information, emergency contact, medical conditions, and team details.
REQUIRED
2. BIRTH CERTIFICATE
To verify age, identity, and division eligibility.
Player must be the correct age for the division. Players may play up, but may not play down.
REQUIRED
3. SCHOOL DEMOGRAPHIC FORM
Verify school, grade, parent/guardian information, and demographic details.
REQUIRED
4. SPORTS PHYSICAL / MEDICAL CLEARANCE
Completed by a licensed medical provider.
Must be current and state the athlete is cleared for sports participation.
REQUIRED
5. LIABILITY WAIVER / RELEASE OF LIABILITY
Acknowledgement of risk, assumption of risk, release of liability, and emergency medical authorization. Must be signed by parent/guardian.
REQUIRED
6. MEDICAL INSURANCE &
EMERGENCY TREATMENT RELEASE
Parent/guardian confirms medical insurance or accepts responsibility for medical expenses.
Includes emergency treatment authorization.
REQUIRED
7. PLAYER CODE OF CONDUCT
Player agrees to respect coaches, teammates, officials, opponents, and follow league rules.
Includes behavior expectations and consequences.
REQUIRED
8. PARENT / GUARDIAN CODE OF CONDUCT
Parent/guardian agrees to support the league, respect officials, coaches, and players, and follow facility and league rules.
REQUIRED
9. MEDIA RELEASE FORM
Authorizes the league and Valley Life Media Group to photograph, record, interview, and use player images and stories for promotional purposes.
REQUIRED
10. CONCUSSION INFORMATION SHEET
California law requires annual concussion information.
Must be signed by athlete and parent/guardian.

11. SUDDEN CARDIAC ARREST
INFORMATION SHEET
California law requires annual sudden cardiac arrest information. Must be signed by athlete and parent/guardian.
REQUIRED
12. EMERGENCY MEDICAL AUTHORIZATION
Provides emergency contacts, medical conditions, allergies, medications, and authorizes the league to seek emergency medical treatment.
REQUIRED
PARTICIPATION DISCLAIMER
Submitting registration documents does not automatically approve a player for participation. All documents must be reviewed and approved by Valley Girls Flag Football League.
Players may not participate in practice, games, playoffs, media events, or championship activities until their repistration file is complete and approved.
